As the participant and/or guardian of the participant, I hereby grant permission to the Pathways Program to post my and/or my child's story, photos, audio, video or other items, hereinafter referred to as "Materials.” I agree that Pathways Program may use and authorize the use of any part of this documentation for, but not limited to, exhibition, publication, educational and website purposes. Additionally, I waive any right to royalties or other compensation arising or related to the use of these “Materials,” or any right therein.

By selecting “YES” in the registration form under “The Media Consent and Release”, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against Pathways Program utilizing these “Materials” for the above purposes.
